이탈리아 코로나바이러스(Covid19)사망자 많은 이유?(ENG Why Coronavirus is serious in Italy)

이탈리아 코로나 사태가 심각합니다.

이탈리아 코로나19 첫 확진자는 우한을 출발한 중국인 관광객 부부였구요.

이탈리아 정부는 즉시 비상사태를 선언하고 이탈리아와 중국을 오가는 모든 항공 직항편을 중단시켰지만, 특정 국적자가 경유 편으로 입국하는 것을 막지는 않았어요.

오늘은 이탈리아에서 코로나19가 급속하게 확산되는 원인을 한번 살펴보도록 하겠습니다.

인구통계학

이탈리아는 세계2위의 고령자 인구 구성을 가진 국가죠.  이탈리아의 65세이상 인구 비율이 20%이상으로 일본에 이어 세계 2위구요. 사망자의 절대 다수가 기저질환이 있는 63세∼95세 사이의 노년층이며, 사망자 평균 나이는 81세라고 합니다. 이탈리아에서의 코로나19 치사율은 3.15%로 한국(0.59%)의 6배에 달하죠.

현지 전문가들은 그 원인을 세계적으로 높은 고령 인구 비율에서 찾고 있습니다.

가족유대감

이탈리아에서는 한때 ‘총각세’를 내게 하자는 말이 나올 정도로 젊은이들이 결혼하지 않고 아이도 갖지 않죠.

이탈리아어로 ‘맘모니( Mammoni)’란 단어를 아십니까? 마마보이란 뜻이에요.

이탈리아는 가족 간의 유대가 우리나라보다 더 강한 나라인지도 모르겠어요. 

특히 엄마와 아들 간의 돈독한 관계로 인해서 이탈리아에서는 마마보이로 불리는 남자들이 많죠. 그들은 창피하게 여기지도 않아요. 그들은 경제적으로 자립했는데도 엄마랑 살아요. 편하기 때문이죠. 왜냐하면 엄마는 요리며 빨래며 아들 뒷바라지를 해주고 있으니까요.

이탈리아의 18-40세 성인 중 약 48%의 남성이 아직 부모와 함께 살고 있는 것으로 나타나고 있습니다.

이들은 도시에 나가 일하면서도 외곽 근교 부모님집에서 출퇴근하다 보니까 젊은이들이 나이 드신 부모님과의 접촉이 빈번할 수밖에 없는 거죠. 자각증세가 없는 상태에서 부모님들께 코로나바이러스를 전염시킬 가능성이 높아지고 있다고 분석하고 있습니다.

신체접촉문화

이탈리아인들은 낙천적이고 활발한 성격에 사교와 놀이 문화가 많이 발달되어있죠.  허그와 볼키스와 같은 인사문화로 서로간에 친밀감을 나타내다 보니까 사회적 거리 두기 조치가 제대로 시행되기 힘든 문화를 가지고 있다고 하겠어요.

주세페 콘테 총리는 국민 담화를 통해서 감염을 촉발할 가능성이 있는 빈번한 외식이나 밤에 함께 술 마시고 노는 생활, 신체 접촉을 유발하는 인사 습관을 바꿔야 한다고 촉구했는데요. 글쎄요 단기간에 얼마만큼 이탈리아 문화가 바뀌게 될지는 잘 모르겠습니다.

안전 불감증

이탈리아 국민들이 초기에 코로나19에 무관심하거나 그다지 심각하게 생각하지 않았다고 하네요. 초기에 자가격리 조치도 제대로 취하지 않았구요. 서구사회에서는 마스크를 쓰는 사람은 자신의 정체성을 감추는 일이기 때문에 이상한 사람으로 보게 되는 경향이 많죠.  대부분 마스크를 쓴 사람은 강도나 테러범이었기 때문에 마스크 착용에 거부감을 느끼고 있는 문화를 가지고 있죠.

유럽에는 마스크를 사용하는 사람이 없으니까 마스크가 필요가 없고 마스크를 만드는 공장이 별로 없는 거죠. 대부분 아시아국가에서 마스크를 만들어서 수출을 하는데 현재는 아예 구할 방법이 없는 상태죠. 밀폐된 공공장소에서 마스크를 사용하지 않는 것도 전염병 확산 요인이 되고 있습니다.

중국인 왕래지역

이탈리아의 경제와 금융 중심지인 북부가 코로나19의 진원지인 중국에서 온 사람들의 왕래가 많은 지역이라고 하네요. 실제로 2008년 경제 위기 이후 중국인들이 이탈리아 기업을 대거 인수하고, 이탈리아 당국이 재정 확보를 위해 중국인 관광객을 적극적으로 유치하게 된거죠. 중국인의 이탈리아 왕래는 크게 늘었죠. 이탈리아의 연간 관광객 수 5800만 중 중국인이 350만명이고, 작년 2019년에는 600만명에 달한다는 조사 결과도 있습니다. 

이번 춘제에도 많은 중국 화교가 고향에 다녀왔을 것이라는 추측이 일어나고 있죠.

이탈리아에는 유럽 통계상 32만 명, 합법·비합법 거주자를 합하면 40만 명에 이르는 중국인과 중국계 주민이 거주하고 있다고 합니다. 이탈리아는 G7 회원국 중 유일하게 2019년 3월 23일 중국과 일대일로 양해각서(MOU)에 서명해서 협력 관계를 맺었죠. 일본의 요미우리 신문은 “경제적 상호의존 관계도 감염 확대의 배경일 가능성이 있다”고 지적하고 있습니다.

소극적 정부 대응

2020년 2월21일 이탈리아 북부에서는 중국을 방문한 적이 없는 38세의 남성이 최초로 양성반응을 보였죠. 이 남성은 확진 판정이 나기 3일 전에 이미 독감 같은 증상으로 입원한 아내와 몇몇 의사, 간호사, 환자들을 감염시킨 것으로 확인됐구요. 전문가들은 북부 지역에서 최소한 1월 하반기부터 코로나19 바이러스가 존재해 퍼졌을 것으로 보고 있습니다.

정부가 외국에서 들어오는 중국인이나 중국인 접촉자들에 대한 방역에만 중점을 두다가 정작 지역사회 감염을 놓쳤다는 평가가 나오고 있는 이유죠. 감염 확대를 저지해야 할 정부가 초기 대응에 늑장을 부리는 바람에 사태가 커졌다고 비판하는 목소리가 거세지고 있습니다

의료 시스템

이탈리아는 중국이나 이란과 달리 선진적인 공중보건 및 의료 시스템을 보유한 국가인데 왜 이렇게 높은 치사율을 갖고 있냐고 의문점을 갖고 있는 사람이 많죠. 이탈리아는 유럽의 다른 국가들처럼 전 국민은 물론 외국인 거류자에게도 ‘보편적 의료복지’를 제공합니다. 이탈리아 의료비에서 개인 부담 비율은 23%로 EU 평균 15%보다 8%가 높은 편이죠.

개인 부담이 높다는 것은 마음대로 의료 서비스를 이용하는 데 한계가 있다는 뜻입니다. 이탈리아정부의 현금 살포성 포퓰리즘으로 국가채무가 무려 3230조원에 이르다 보니까 보건 의료분야에 투자할 여력이 없는거죠. 

정부주도의 공공 서비스로 의료 부문을 운영하다 보니 의료장비가 부족하고 인력 수준이 저하될 수밖에 없는 상태입니다.

젊은 의사들은 공무원 신분을 거부하고 인근 유럽나라로 이민을 가서 10여년간 이민간 의사가 약 만명에 달한다고 하는군요. 재정적자 폭을 줄이기 위해 유럽연합(EU)의 기준에 맞춰 이탈리아정부는 긴축재정을 하게 되죠. 지난 5년 간 전국의 병원 등 의료기관 758개소가 문을 닫게 됩니다.

인력도 대폭 감축된 상태에서 전염병 공격을 당한 셈이죠.

코로나19로 전 국민이 집단으로 자가격리에 들어간 이탈리아인들은 창문을 열고 베란다에 서서 차례로 노래하기도 하고, 프라이팬과 스테인리스 그릇을 두들기거나, 함성을 지르고 손뼉을 치면서 서로 격려한다고 하는군요.

창가에 ‘안드러 투토 베네(Andra Tutto Bene) ‘모든 일이 잘 될 거야’ 라고 적힌 팻말을 들기도 하면서요.

온세계가 겪는 코로나바이러스 사태가 하루빨리 종식되어 모든 일이 잘 되길 기도해봅니다.

The Italian corona crisis is serious.

The first confirmed corona19 in Italy was a Chinese tourist couple from Wuhan.

The Italian government immediately declared an emergency and stopped all direct flights to and from Italy and China, but did not prevent certain nationals from entering via transit.

Today we will look at the causes of the rapid spread of Corona 19 in Italy.

Demographics

Italy is the world's second-largest population. Italy is the second largest in the world after Japan with a population of over 20% over 65 years old. 

The vast majority of deaths are between the ages of 63 and 95 with underlying disease, and the average age of death is 81.

The Corona 19 mortality rate in Italy is 3.15%, six times that of Korea (0.59%).

Local experts are looking for the cause in the world's highest proportion of the elderly.

Family bond

In Italy, young people don't get married and they don't have children to the point where it was once said to have a 'single tax.'

Do you know the word 'Mammoni' in Italian?  It means Mama boy. Italy may have a stronger family ties than Korea.

In particular, there are many men called Mamaboys in Italy because of the strong relationship between mother and son.

They don't even feel embarrassed. They live with their mother even though they are economically independent. Because it's comfortable. Because their  mom is cooking and washing their son' clothes and doing her son's house chores. About 48% of Italy's 18-40 year olds are still living with their parents.

As they go out to work in the city and commute from their outskirts of their parents' homes, it is inevitable that young people will have frequent contact with older parents.

It is analyzed that the likelihood of transmitting the coronavirus to parents is increasing in the absence of awareness of Covid-19.

Body contact culture

Italians are optimistic and active, with a lot of social and play culture. Since they show intimacy with each other through a greeting culture such as Hug and cheek kiss, I would say that it has a culture where it is difficult to properly implement social distance measures.

Prime Minister Giuseppe Conte has recommnemded that they should not frequently eat out , drink and play together at night, which may trigger an infection through public speech. 

He urged them to change the personnel habits that cause physical contact. Well, I'm not sure how much Italian culture will be changed in the short term.

Safety insensitivity

Italians are initially indifferent to Corona 19 or not so serious  about the virus.

Initially, self-containment measures were not taken properly.

In Western society, people who wear masks tend to see them as strange because they hide their identity.

Most people who wear masks are robbers or terrorists, so they have a culture of refusal to wear masks.

In Europe, there are no people using masks, so there is no need for masks and there are not many factories that make masks.

Most Asian countries make and export masks, but there is currently no way to get them.

The culture that they do not use masks in closed public spaces causes an epidemic.

Chinese traffic zone

It is said that the northern part of Italy's economic and financial center is the region where people come and go from China, the epicenter of Corona 19.

In fact, after the 2008 economic crisis, the Chinese took over a large number of Italian companies, and the Italian authorities actively attracted Chinese tourists to secure finances.

The Chinese traffic to Italy has increased significantly.

According to a survey, it is estimated that 3.5 million Chinese are among the 58 million tourists annually in Italy and 6 million in 2019.

There is speculation that many Chinese people living in Italy visited their hometowns this Lunar New Year's Day.

In Italy, according to European statistics, there are 320,000 Chinese and 400,000 Chinese and Chinese residents, including legal and illegal residents.

Italy was the only member of the G7 to sign a Memorandum of Understanding (MOU) with China on March 23, 2019, in a partnership.

The Japanese Yomiuri Newspaper pointed out that "economic interdependence is also likely to be the background of the spread of infection."

Passive government response

On February 21, 2020, in northern Italy, a 38-year-old man who had never visited China had the first positive test.

The man was confirmed to have infected his wife, some doctors, nurses, and patients who had been hospitalized for flu-like symptoms three days before the confirmation.

Experts believe that the corona19 virus has been present in the northern region since at least the second half of January.

That's why the government is only evaluating the outbreak of community infections, focusing only on the prevention of Chinese or Chinese contacts from abroad.

There is a growing voice of criticism that the situation has grown because the government, which has to stop the spread of infection, is procrastinating against the initial response.

Medical system

Italy, unlike China and Iran, has advanced public health and health systems, and many people have questions about why they have such a high mortality rate.

Italy, like other countries in Europe, provides 'universal health care' to both foreign and foreign residents. The share of personal burden in Italian medical expenses is 23%, 8% higher than the EU average of 15%.

The high personal burden means that there are limits to using medical services at will. With the Italian government's cash-spreading populism, the national debt amounted to 3,230 trillion won, so there is no room for investment in health care.

Running the medical sector with government-led public services means that there is a shortage of medical equipment and manpower levels are inevitably reduced.

Young doctors refused to be government  officials and immigrated to neighboring European countries, saying that there were about 10,000 doctors who immigrated for 10 years.

In order to reduce the budget deficit, the Italian government is tightening to meet EU standards. Over the past five years, 758 medical institutions including hospitals and clinics have been closed. As manpower was reduced, the coronavirus seriously hit Italy.

Italians who committed self-isolation because of Corona 19 open the window and sing in turn on the balcony.

It is said that they encourage each other by tapping frying pans and stainless steel bowls, shouting and clapping their hands.

They even hang a sign at the window saying, 'Andra Tutto Bene,' All will be fine '.

I pray that the whole world  with Coronavirus will end quickly and everything will work out.
